PATIENT FINANCING
Fees and Payment:
We make every effort to keep your medical care cost to a minimum and to provide you the most convenient financial options. Payment is due at the time service is rendered. This is expected unless prior financial arrangements are made in advance. We are happy to arrange with you a flexible payment plan. For your convenience we accept personal checks, Visa, MasterCard, Discover and American Express. We can also arrange payment for our services through a patient financing service called Care Credit that is designed specifically for oral and maxillofacial surgery that provides funds for your services at a very reasonable rate.
Insurance:
We make every effort to offer you the most convenient financial options. As a convenience to you, we will assist you to maximize your insurance reimbursement for covered procedures by filling your insurance claim for you. However, you are responsible for the deductibles, co-pays and office visit fees outlined in your particular insurance plan. Please bring your insurance information with you on the date of your initial consultation. If your insurance company requires a referral number before seeing a specialist, please bring it with you on the day of your visit or your claim may be denied.
