Making Appointments

We're pleased to offer you the ability to make your appointments online. Please fill out the below appointment request form, and one of our staff will get back with you within one business day to schedule your appointment.

Appointment Request

The Center for Oral and Maxillofacial Surgery is open Monday - Friday from 8:30 am to 5:00 pm. We will schedule your appointment as promptly as possible. If you find it neccessary to cancel your appointment, we would appreciate 24 hours notice. If you have pain or an emergency situation, every effort will be made to see you that day. If you have an emergency after hours, please call (706) 724-8735 and your message will be forwarded to one of our doctors on-call.

At the time of your initial consultation, please bring with you x-rays from your referring dentist and your referral slip and/or insurance information. Please note, all patients under the age of 18 must be accompanied by a parent or guardian at the consultation appointment.
